Water Aerobics and Tinnitus: Health Benefits and What Research Says
Living with tinnitus can affect more than hearing. For some people, the persistent perception of ringing, buzzing, humming, hissing, or other sounds can interfere with concentration, sleep, mood, and overall quality of life. [1]
Regular physical activity is an important part of maintaining overall health, and water aerobics offers an accessible way to exercise while taking advantage of the unique properties of water. [3]
But can water aerobics actually help someone with tinnitus?
The answer requires an important distinction: water aerobics is not a treatment or cure for tinnitus, and current scientific evidence does not establish aquatic exercise as a tinnitus therapy. [1–3]
However, research suggests that physical activity is associated with better quality of life and lower tinnitus distress in people with tinnitus. [2] Separately, substantial research on aquatic exercise has demonstrated benefits for physical fitness and certain measures of psychological well-being. [3,4]
That makes water aerobics an interesting option for supporting overall physical and emotional wellness while living with tinnitus—without claiming that exercising in water will eliminate the tinnitus itself.
What Is Tinnitus?
Tinnitus is the perception of sound without an external sound source. People commonly describe it as ringing in the ears, although it may also sound like buzzing, roaring, clicking, humming, hissing, or squealing. [1]
According to the National Institute on Deafness and Other Communication Disorders (NIDCD), tinnitus is common, with surveys estimating that approximately 10% to 25% of adults experience it. [1]
Tinnitus is strongly associated with hearing loss, although it can have many potential contributing factors, including noise exposure, certain medications, ear conditions, head or neck injuries, and some underlying health conditions. [1]
Importantly, tinnitus is not experienced the same way by everyone.
For some people it is relatively unobtrusive. For others, it can interfere with sleep, concentration, mood, and everyday activities. Severe tinnitus may also be associated with anxiety or depression. [1]
There is currently no universal cure for tinnitus. Management may include approaches such as hearing aids when hearing loss is present, sound therapy, counseling, cognitive behavioral therapy, and treatment of an identifiable underlying cause when appropriate. [1]
What Is Water Aerobics?
Water aerobics is a form of exercise performed while partially submerged in water, usually in a swimming pool.
Classes can incorporate movements such as:
- Water walking or jogging
- Leg lifts
- Knee raises
- Arm movements
- Resistance exercises
- Balance exercises
- Low-impact jumping or stepping movements
- Aqua cycling or other water-based cardiovascular activities
Water creates resistance as the body moves through it while buoyancy changes the mechanical demands placed on the body.
As a result, water-based exercise can provide cardiovascular, strength, balance, and flexibility training while offering an alternative to conventional land-based workouts. [3]
The Proven Health Benefits of Aquatic Exercise
There is considerably more scientific evidence supporting aquatic exercise for general health and fitness than there is for tinnitus specifically. [2,3]
A systematic review and meta-analysis published in the Scandinavian Journal of Medicine & Science in Sports evaluated 62 studies involving healthy adults and adults with chronic diseases. [3]
Among healthy adults, water-based training produced benefits in areas including strength, balance, and cardiorespiratory fitness. [3]
The researchers also identified improvements in various health and fitness measures among populations with certain chronic conditions, although the specific benefits differed depending on the condition being studied. [3]
The authors concluded that water-based training can be an effective form of physical activity for improving health and physical fitness. [3]
That makes aquatic exercise valuable on its own—regardless of whether someone has tinnitus.
So How Could Water Aerobics Be Relevant to Tinnitus?
This is where the evidence needs to be interpreted carefully.
There is evidence suggesting that physical activity in general may be beneficial to quality of life among people with tinnitus. [2]
One notable study published in Ear and Hearing examined physical activity, tinnitus severity, and quality of life. [2]
Researchers found that higher levels of physical activity were significantly associated with:
- Better health-related quality of life
- Better overall quality of life
- Lower tinnitus severity [2]
However, the relationship between physical activity and tinnitus distress was relatively small, and this was an observational survey rather than a randomized clinical trial. [2]
That means the research demonstrated an association. It did not establish that exercise caused tinnitus to improve.
The researchers themselves concluded that physical activity may potentially have a role in tinnitus management but that additional research is necessary. [2]
Even more importantly, the study investigated physical activity generally—not water aerobics specifically. [2]
Therefore, it would be scientifically inappropriate to conclude from this study that water aerobics treats or reduces tinnitus.
A more evidence-based interpretation is:
Water aerobics provides a way to participate in regular physical activity, and physical activity has been associated with better quality of life and lower tinnitus distress. [2,3]
1. Water Aerobics May Support Emotional Well-Being
One reason exercise may be particularly relevant for someone living with tinnitus is the relationship between tinnitus and emotional well-being.
NIDCD notes that tinnitus can affect mood and, in severe cases, may contribute to anxiety or depression. [1]
Water-based exercise has also been studied for its psychological effects. [4]
An integrative review published in Geriatric Nursing evaluated 18 studies of water-based exercise in adults age 60 and older. [4]
The review found evidence suggesting that water-based exercise can benefit measures including:
- Quality of life
- Mood
- Depression
- Anxiety
- Tension
- Confidence related to falling [4]
These findings don’t mean that water aerobics treats tinnitus-related anxiety or depression.
Rather, they suggest that aquatic exercise may support aspects of psychological well-being that can be especially important for people managing a chronic and sometimes intrusive condition such as tinnitus. [1,4]
2. It May Help Support Overall Quality of Life
Tinnitus severity and quality of life are closely connected. [1,2]
In the Ear and Hearing study examining people with tinnitus, greater physical activity was associated with better health-related and overall quality of life. [2]
Separately, aquatic-exercise research has found improvements in quality-of-life measures in a variety of populations. [3,4]
For example, the large systematic review and meta-analysis of aquatic exercise found improvements in quality-of-life measures among some groups with chronic health conditions. [3]
This does not establish water aerobics as a tinnitus intervention.
It does, however, support a broader idea: regular movement and physical fitness can be valuable parts of maintaining quality of life while living with a chronic condition.
3. Water Provides a Different Exercise Environment
Water aerobics has another advantage: it provides a different mechanical environment from exercising on land.
Water provides resistance to movement while buoyancy reduces the amount of body weight supported during many movements.
This can make aquatic exercise an appealing alternative for people who find certain forms of land-based exercise uncomfortable or difficult.
A large systematic review found benefits from water-based training in strength, balance, cardiorespiratory fitness and other physical-fitness measures, depending on the population studied. [3]
For someone with tinnitus, the relevance is indirect but potentially important.
Finding an enjoyable and sustainable form of exercise can make maintaining an active lifestyle easier—and consistency is one of the most important elements of any exercise program.
4. Water Aerobics Can Support Cardiovascular Fitness
Water aerobics isn’t simply relaxation in a swimming pool.
Depending on the intensity of the workout, it can provide genuine aerobic exercise.
The systematic review and meta-analysis of aquatic exercise found improvements in cardiorespiratory fitness among healthy adults participating in water-based training. [3]
This matters for overall health because cardiovascular fitness is an important component of physical health.
There is also an important tinnitus distinction here.
NIDCD notes that certain blood-vessel problems—including high blood pressure, atherosclerosis, and vascular malformations—can sometimes be associated with tinnitus, particularly when blood flow near the ear is involved. [1]
That does not mean improving cardiovascular fitness will cure tinnitus, nor should someone attempt to treat pulsatile tinnitus through exercise.
Tinnitus that beats or pulses in rhythm with the heartbeat deserves medical evaluation because it can have an identifiable underlying cause. [1]
5. It Can Provide Strength and Balance Benefits
Aquatic exercise isn’t limited to cardiovascular activity.
Moving arms and legs against the resistance created by water can challenge muscles throughout the body.
Research examining water-based exercise has found improvements in strength and balance in healthy adults, while benefits have also been reported in certain populations with chronic health conditions. [3]
These are general physical-fitness benefits rather than tinnitus-specific effects.
But maintaining strength, mobility, balance, and physical independence contributes to overall well-being—and those benefits remain worthwhile for someone who happens to live with tinnitus.
An Important Consideration: Tinnitus and Balance Problems
Some people with tinnitus also experience dizziness or balance problems. [1]
That doesn’t necessarily mean tinnitus itself is causing the balance problem.
Certain inner-ear disorders can produce combinations of tinnitus, hearing changes, dizziness, and balance symptoms. [1]
For example, NIDCD identifies Ménière’s disease as an inner-ear disorder that can involve tinnitus, hearing loss, and balance problems. [1]
Someone experiencing significant dizziness, vertigo, fainting, or unexplained balance difficulties should therefore seek medical guidance before beginning a water-exercise program.
Water should not be considered automatically safer for someone with a balance disorder.
Entering and exiting a pool, slippery surfaces, water depth, and sudden dizziness can all create safety concerns.
Individuals with balance problems may require medical clearance, supervision, an appropriate pool environment, or another form of exercise.
Protect Your Hearing Around the Pool
People with tinnitus should also pay attention to the sound environment in which they exercise.
Some aquatic fitness classes use amplified music and instructors may use speakers or microphones to communicate over splashing, ventilation systems, and other background noise.
NIDCD identifies excessive noise exposure as a cause of noise-induced hearing loss and notes that tinnitus can accompany hearing damage. [5,6]
If the music in a class feels excessively loud, consider moving farther from the speaker, asking whether the volume can be reduced, choosing a quieter class, or using appropriate hearing protection when recommended by a hearing professional. [5,6]
Exercise should support your health—not unnecessarily expose your hearing to potentially damaging sound levels.
Can Water Aerobics Make Tinnitus Go Away?
Based on current evidence, there is not sufficient scientific evidence to say that water aerobics makes tinnitus disappear, cures tinnitus, or directly treats its underlying cause. [1–3]
That distinction matters.
Research has found an association between greater physical activity and lower tinnitus severity and better quality of life. [2]
Research has also independently demonstrated health and fitness benefits from aquatic exercise. [3,4]
But combining those findings does not prove that water aerobics is a tinnitus treatment.
More research would be needed to determine whether aquatic exercise has a specific therapeutic effect on tinnitus.
A Healthy Way to Think About Water Aerobics and Tinnitus
Instead of asking:
“Will water aerobics cure my tinnitus?”
A more evidence-based question may be:
“Can water aerobics help me maintain my physical fitness and overall well-being while I live with tinnitus?”
For many people who are medically able to exercise, the answer may be yes.
Water aerobics can provide an enjoyable form of physical activity with evidence supporting benefits for areas such as strength, balance, cardiorespiratory fitness, and—in certain populations—quality of life and psychological well-being. [3,4]
Those benefits can matter whether or not tinnitus itself changes.
How Often Should You Do Water Aerobics?
There is no scientifically established water-aerobics prescription specifically for tinnitus. [1–3]
In a systematic review of aquatic exercise, beneficial water-based training programs among healthy adults commonly involved training for at least 12 weeks, approximately two to three times per week, with sessions lasting roughly 46 to 65 minutes. [3]
Those figures describe patterns found in the research and should not be interpreted as a personal medical prescription.
The appropriate duration and intensity of exercise varies according to age, fitness, cardiovascular health, medications, mobility, balance, and other medical considerations.
Someone beginning an exercise program can consider starting gradually and increasing activity according to their ability and guidance from an appropriate healthcare professional.
The Bottom Line
Water aerobics does not currently have sufficient evidence to be considered a treatment or cure for tinnitus. [1–3]
What the science does support is more nuanced.
Aquatic exercise can provide meaningful general health and physical-fitness benefits. Research has demonstrated benefits involving strength, balance, cardiorespiratory fitness and quality of life in various populations. [3,4]
Separately, research involving people with tinnitus has found that higher physical-activity levels are associated with better quality of life and slightly lower tinnitus distress. [2]
Water aerobics can therefore be viewed as one way of supporting an active, healthy lifestyle while living with tinnitus—not as a replacement for evidence-based tinnitus care or medical evaluation.
If tinnitus is new, persistent, worsening, occurs predominantly in one ear, pulses with the heartbeat, or is accompanied by sudden hearing changes, significant dizziness, weakness, or other concerning symptoms, seek evaluation from an appropriate healthcare professional. [1]
